Key Responsibilities

• Provide physical therapy services to patients in a home health setting according to a physician's written plan of care
• Assess, evaluate, and update patients' therapeutic, rehabilitative, and functional status; participate in the development of the total plan of care
• Evaluate home environments and assess muscle strength, mobility, gait, and range of motion; direct physical therapy treatment accordingly
• Instruct patients, families, and caregivers in the use and care of therapeutic appliances and prosthetic devices as applicable
• Prepare and submit clinical and progress summaries; report changes in patient condition to the attending physician and document in the patient health record
• Supervise Home Health Aides participating in a patient's rehabilitation regimen and identify needs for additional home health services, referring as necessary

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Minimum of 2 years of experience as a PT in an acute, home care, or rehabilitation setting
• Current BLS certification
• Strong oral and written communication skills with solid organizational abilities
• Ability to travel throughout the assigned service area within Yolo County and surrounding communities

Preferred Experience / Skills

• Familiarity with Title 22, Medicare Conditions of Participation, Medi-Cal, and OSHA regulations as they apply to home health
• Experience participating in case conferences and peer consultation processes
• Prior home health experience with demonstrated ability to manage an independent caseload
