Job Summary:
The organization is seeking a Physical Therapist specializing in skilled nursing facility (SNF) settings in HILO, Hawaii. This role involves providing rehabilitation services including evaluations, treatments, and development of individualized care plans, ensuring compliance with state and federal regulations. The position requires working primarily days, with potential flexibility including one weekend day, and may involve floating to other units or facilities as needed.

Job Details:

• Schedule: Generally scheduled for five days per week, with one weekend day; shift hours are 8:00 AM to 4:00 PM, requiring a minimum of 40 hours per week.
• Required Qualifications: Current State of Hawaii license in good standing appropriate to the discipline; Basic Life Support (BLS) certification; minimum two (2) years of experience in the SNF/ICF setting preferred; must possess an active Hawaii license by the start of the assignment.
• Preferred Qualifications: Experience performing evaluations, reassessments, treatments, and developing individualized treatment plans.
• Additional Information: Candidates must arrive the Thursday before the start date to attend employee health appointments scheduled for Friday. This role involves providing evidence-based therapeutic interventions to maximize patient function and quality of life, as well as supervising therapy assistants when applicable.
• Submission Requirements: Candidates should be prepared to demonstrate licensure and certification upon acceptance and adhere to all facility operational policies.
