Job Summary:
Physical Therapist (PT), Skilled Nursing Facility (SNF) Oklahoma City, OK. This role involves effective screening, evaluation, and rehabilitation of patients with physical functioning disorders and wound care needs. The candidate will develop and implement personalized care plans, utilize standardized assessments, and administer procedures such as manual techniques, therapeutic exercises, and wound care. Providing patient and caregiver education, evaluating treatment outcomes, and documenting all clinical activities compliant with regulatory standards are integral responsibilities.

Job Details:

  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week
  • Required Qualifications: Valid Oklahoma state license, 1 year of physical therapy experience in long-term care or rehabilitation settings, graduate degree in physical therapy, ability to obtain necessary DME or supplies per clinical standards
  • Preferred Qualifications: Experience supervising physical therapist assistants, rehabilitation aides, or students; familiarity with APTA and State practice acts, code of conduct, and ethics
  • Additional Information: Effective communication with multidisciplinary teams regarding patient progress, barriers, and treatment plans. Participation in meetings such as patient care conferences, utilization reviews, and rehab meetings is expected. The candidate must comply with all regulatory and clinical documentation requirements and participate in required trainings and conferences.
