Key Responsibilities- Provide physical therapy assistant services to patients in their homes and in care facilities across the assigned territory, including Monroe County
- Manage a caseload of approximately 5-10 patients per day, with an expected productivity target of 30 points per week (equivalent to approximately 40 hours per week)
- Conduct Start of Care visits and Follow-Up visits as assigned
- Complete all clinical documentation within 24 hours of each patient appointment using HomeCare HomeBase EMR and OASIS
- Travel up to 75 miles from the home clinic to reach patients; mileage is reimbursed at the IRS standard rate from patient home to patient home
- Attend periodic office meetings at the assigned care center in addition to daily home visits
Required Qualifications- Active, unrestricted Physical Therapy Assistant (PTA) license - must be in hand at time of submission (no pending licenses accepted)
- Minimum 1 full year of recent Adult Home Health experience - first-time home health candidates will not be considered
- CPR/BLS certification required
- Reliable personal vehicle, valid driver's license, and agency-required auto liability insurance
- Proficiency in HomeCare HomeBase EMR within the last 6 months, or willingness to complete a retraining course during compliance
- Must be willing and able to travel up to 75 miles from the assigned care center and accept all patients on the assigned schedule regardless of distance from previous appointment
Preferred Experience / Skills- Experience with OASIS documentation, used within the last 6 months
- Familiarity with a productivity/points-based caseload model
- Experience covering multi-county or wide geographic territories in a home health setting
