Key Responsibilities

• Provide direct physical therapy or physical therapy assistant services to residents in a skilled nursing facility setting
• Evaluate, plan, and implement individualized treatment programs to improve patient mobility, strength, and functional independence
• Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning

Required Qualifications

• Active state licensure as a Physical Therapist (PT) or Physical Therapist Assistant (PTA) in Wisconsin
• Current CPR/BLS certification
• Ability to work day shift hours

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care environment
• Familiarity with Medicare documentation and compliance requirements
