Responsibilities
- Administer skilled nursing care to home health patients in their residences and coordinate care with the interdisciplinary team, patient/family, and referring agency.
- Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document findings in the medical record and care plans.
- Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity of care; update as conditions change.
- Obtain data on factors influencing health status and incorporate findings into the Plan of Care; maintain accurate and timely patient records.
- Provide care using infection control practices; educate patients and families on care, medications, safety, and self-care practices; adapt teaching to age, condition, culture, and readiness.
- Supervise and provide clinical direction to Home Health Aide / STNA / LVN as needed to ensure quality of service; coordinate referrals to appropriate agencies when indicated.
- Monitor and reassess pain; apply pain management techniques and educate patients/families on pain control strategies.
- Communicate effectively with patients, families, team members, and physicians; participate in interdisciplinary team meetings as appropriate.
- Perform on-call responsibilities as assigned and ensure appropriate documentation and follow-up.
Requirements
- Current Registered Nurse license in Washington – Required
- CHPN certification – Preferred
- CPR certification – Required
- 1–2 years of RN clinical care experience; 2+ years of home health experience – Required
- OASIS Experience – Required
- Homecare Homebase Software Experience – Required
- Education: ADN or Nursing Diploma – Required; BSN – Preferred
- Current driver's license – Required; reliable transportation and insurance – Required
- Ability to travel locally to patient homes; maintain confidentiality; strong organization and communication skills
