Position Title: Travel Registered Nurse (RN) – Case Manager
Location: Eureka, CA 95501
Assignment Duration: 13 Weeks
Schedule: Night Shift – 5x8 Hours (7:00 PM – 7:00 AM)

Position Overview

"Navitas Healthcare, LLC" is seeking a skilled and compassionate Registered Nurse (RN) – Case Manager for a 13-week travel assignment in an acute care setting. This role is ideal for professionals with strong clinical expertise, care coordination experience, and a commitment to delivering high-quality patient outcomes.

Key Responsibilities

• Coordinate and oversee patient care plans across the continuum of care
• Collaborate with interdisciplinary teams to ensure effective treatment and discharge planning
• Conduct utilization reviews and monitor patient progress
• Adjust care plans based on clinical assessments and patient needs
• Ensure compliance with regulatory standards and documentation requirements
• Utilize electronic medical records (Epic) for charting and coordination
• Educate patients and families on care plans, discharge instructions, and available resources

Required Qualifications

• Active Registered Nurse (RN) license
• Minimum 2 years of acute care experience (required)
• Prior case management experience preferred
• First-time travelers considered (based on clinical evaluation)
• Strong communication, organizational, and critical thinking skills
