PPOSITION SUMMARY – RN Case Manager/Care Coordination 

POSITION DUTIES – Coordinate patient care across providers, oversee utilization review to ensure appropriate and cost-effective services, and facilitate timely, safe discharge planning by arranging post-acute resources and support.

MINIMUM REQUIRED QUALIFICATIONS – 

• CA RN license
• BLS

PREFERRED QUALIFICATIONS – Minimal time off requests

LENGTH OF ASSIGNMENT – 17 weeks

SHIFT / HOURS PER WEEK – 8hr Days, EOW

SYSTEMS – Cerner

START DATE – ASAP

General Comments

TYPE OF JOB ORDER – Travel

REQUIRED SKILLS – 

• 3 years hospital experience and acute level care coordination experience 
• Skilled experience with discharge planning and utilization review
• Prior CA Dignity/Commonspirit acute care experience in specialty - BUT if you have a strong candidate, I will send file and ask!
