**Exciting Opportunity: RN Case Manager** **Job Description:** - Shift: Monday to Friday, 8am-5pm & Callback on weekends/holidays - Minimum Experience: 3 years - Certifications Required: - ACLS - BLS/BCLS - PALS - NRP - ATT **Position Highlights:** - Become an integral part of a compassionate team focused on delivering exceptional care to culturally diverse patient populations. - Serve as an Outpatient/Inpatient Nurse Case Manager, guiding high-risk patients through their healthcare journey. - Ensure quality, cost-effective healthcare while meeting essential health and social needs. - Facilitate smooth transitions in care from admission to discharge, ensuring continuity and alignment of services. - Collaborate with an interdisciplinary team to assess care plans, monitor progress, and make recommendations for safe and efficient hospitalizations. - Evaluate patients in the Emergency Department for medical necessity and care coordination. - Support individuals with mental health issues in accessing and navigating vital resources to enhance their well-being. - Work to address barriers to self-sufficiency, including illiteracy, domestic violence, and substance abuse, while empowering patients towards better health. **Necessary Qualifications:** - Education: - Associate's or Bachelor's Degree in Nursing - Certifications/License: - Valid, unrestricted RN License to practice in any U.S. state, Puerto Rico, or U.S. territory - Current Basic Life Support (BLS) certification by the American Heart Association - Experience: - 3 years of clinical nursing experience - 1 year in an Outpatient/Inpatient setting - Knowledge of case management, contract health, and utilization review processes is essential. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
