Minimum 2+ years recent Acute Care Case Management/Care Coordination RN experience required / BSN required / CERTIFICATIONS – BLS Required / LICENSE – Active California RN License Required / Schedule: 8-Hour Day Shifts, 40 Guaranteed Hours per Week / Start Date: 07/07/2026 / Duration: 13 Weeks / Setting: Acute Care Hospital, Level II Trauma Center / Unit: Care Coordination/Case Management / EMR: Cerner and InterQual / Skills: Discharge Planning, Utilization Management, Initial Assessments, InterQual Criteria, Care Coordination, Collaboration, Teamwork, Effective Communication, Prioritization Skills, Working with All Payer Sources, Uninsured and Homeless Patient Populations / Patient Volume: 15–20 Patients/Consults per Day / Coverage: All Areas of the Hospital, No Outpatient or Home Care Responsibilities / Weekend Requirement: Every Other Weekend Depending on Staffing, Minimum One Full Weekend per Month / Holiday Requirement: Not Required but Preferred if Willing / Floating: First to Float / Orientation: 12 Hours General Orientation plus 5 Days Care Coordination New Employee Orientation; Additional Unit Orientation Based on Individual Needs / Attire: Business Casual / Hospital Support: Hospitalist In-House, Respiratory Therapy Available 24/7, Pharmacy Not Available 24/7 / Shift Hours: Typically Between 8:00 AM – 5:00 PM Depending on Department / Additional: Both Utilization Review and Discharge Planning Required; InterQual Experience Preferred but Training Available / MUST have recent California Dignity Health experience or candidate will not be considered / Two supervisor references required at submission (charge nurse accepted): one within the last 12 months and one within the last 3 years worked / Candidates must not have been directly employed (FT, PT, or PRN) by any CommonSpirit, CHI, or Dignity Health facility within the past 12 months.
