About the Role

As an RN in Extended Care, you’ll provide holistic, patient-centered care to residents in a long-term care setting. This role focuses on care planning, coordination, and resident advocacy, ensuring each individual reaches their highest level of function and well-being.

 

Key Responsibilities

  • Complete comprehensive assessments & care plans for residents
  • Monitor and evaluate resident conditions and progress
  • Administer medications and IV therapies per protocol
  • Coordinate care with physicians and interdisciplinary teams
  • Lead discharge planning and patient education
  • Supervise and support CNA staff
  • Ensure compliance with CMS, Medicare, and MDS guidelines
  • Respond to urgent and emergent care situations
  • Advocate for residents’ needs, safety, and dignity

 

✅ Requirements

  • Active RN License (State of Practice Required)
  • BLS Certification (AHA Required)
  • ASN or higher from an accredited nursing program
  • Strong documentation and clinical assessment skills
  • Ability to handle physically demanding tasks (lifting, mobility support)
  • Comfortable working in a fast-paced LTC setting
