Job Summary

Physical Therapist SNF. The physical therapist evaluates patients mobility and functional abilities, develops individualized treatment plans, and provides care to reduce pain, restore function, and prevent disability. The role includes ongoing reassessment, patient and caregiver education, interdisciplinary communication, discharge planning, documentation, and participation in quality improvement activities.

Job Details

• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week.
• Required Qualifications: Two years of physical therapist experience; Texas or compact license.
• Responsibilities: Perform patient evaluations, establish individualized plans of care, select appropriate treatment techniques, conduct reassessments, manage discharge planning, supervise and delegate to ancillary staff and students as permitted by applicable laws and policies, complete accurate documentation, communicate with patients and members of the healthcare team, provide patient and caregiver education, participate in professional development and quality improvement activities, and promote a safe care environment.
• Assignment Dates: 11/16/2026 through 02/15/2027.
