Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 5:00 PM | Onsite

Key Responsibilities

  • Provide speech-language pathology services to patients within a home health setting across an assigned territory
  • Conduct evaluations and develop individualized treatment plans addressing speech, language, cognitive, and swallowing disorders
  • Deliver skilled therapy services with an expected productivity of 30 points per week (approximately 6 points per day)
  • Accurately document patient care and progress using HCHB (Homecare Homebase) electronic medical records
  • Collaborate with interdisciplinary care teams to ensure coordinated, patient-centered care

Required Qualifications

  • Active Speech-Language Pathologist (SLP) license in the state of Oregon (or eligibility to obtain)
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or required per facility standards
  • Previous home health experience required
  • Previous HCHB (Homecare Homebase) experience required
  • Ability to travel within a 50-mile radius of the local office

Preferred Experience / Skills

  • Experience managing a caseload independently in a home health or community-based setting
  • Strong time management and organizational skills suited for a field-based role
