Job Summary

Registered Nurse, Wound Care. The position provides specialized skin and wound management for adult and geriatric patients in an outpatient clinical setting. Responsibilities include comprehensive assessment, individualized care planning, patient and family education, care coordination, and collaboration with interdisciplinary healthcare professionals.

Job Details

• Schedule: Monday through Friday, 8-hour day shifts from 8:00 a.m. to 4:30 p.m., with a minimum of 40 hours per week.
• Required Qualifications: Current California registered nurse license; Basic Life Support (BLS) certification; WCC certification; at least 2 years of nursing experience; previous charge experience; community hospital experience; and electronic medical record charting experience.
• Preferred Qualifications: Wound, Ostomy, and Continence Nursing (WOCN) certification; long-term acute care experience; and Level I or Level II trauma experience.
• Additional Information:
  • Provides direct patient skin and wound assessment and management.
  • Develops and coordinates individualized interdisciplinary care plans.
  • Provides patient and family education, including transitions of care.
  • Maintains collaborative communication with patients, support groups, physicians, and healthcare professionals.
  • Serves approximately 810 patients daily, with an acuity of approximately one patient every 30 minutes.
  • Provides care for adult and geriatric patients.
