Job Description
Allegiance Home Health is seeking two detail-oriented and organized Home Health Intake Schedulers to manage the intake process for new patients. This position plays a vital role in ensuring a seamless transition of care for our clients.
Key Responsibilities:
- Schedule initial evaluations and assessments for incoming patients
- New Patient Intake Processing
- Collect and verify accurate patient information, including demographics and insurance details
- Communicate effectively with patients, families, and healthcare providers to coordinate care
- Maintain accurate records and documentation in our electronic health record system
- Collaborate with interdisciplinary teams to optimize patient intake processes
- Provide exceptional customer service and support to patients and families
Qualifications:
- Previous experience in healthcare scheduling or intake is preferred.
- Strong communication and interpersonal skills
- Detail-oriented with excellent organizational abilities
- Proficient in using electronic health record systems and Microsoft Office Suite
- Ability to multitask and work in a fast-paced environment
