Job Description
A nonprofit network in the Bronx, New York City is looking to hire a Wellness Coach to provide case management and hands-on support to adults in a residential program.
About the Opportunity:
Shift: Day
Schedule: Monday to Friday or Tuesday to Saturday
Hours: 9am to 5pm (with one late day required per week from 10:00 AM-6:00 PM)
Setting: Residential
Responsibilities:
Provide case management to an assigned caseload of adult residents
Develop and monitor individual support plans in collaboration with residents
Conduct wellness checks, monthly tenant visits, and apartment inspections as needed
Support residents with problem-solving, crisis intervention, and referrals to community services
Document services, case notes, service plans, and incident-related activity in accordance with agency standards
Qualifications:
Bachelor's degree
At least 2 years of experience working with individuals with Mental Illness, Substance Use issues, and/or a history of homelessness
Working knowledge of email, Microsoft Word, Excel, PowerPoint, and Windows operating systems
Ability to provide case management, referral support, and community outreach in a residential or supportive housing setting
Desired Skills:
Bilingual (English and Spanish)
Experience with Supportive Housing programs
Experience with Group Work, Tenant Organization, and Community Outreach
Familiarity with AWARDS or other electronic documentation systems
Experience supporting adults with Substance Use and Behavioral Health needs
