Wellness Coach - Scatter Site

A nonprofit organization is seeking a Wellness Coach in the Bronx, NY area to provide case management, community outreach, and hands-on support to adults and families with behavioral health needs.

About the Opportunity:

• Schedule: Monday to Friday
• Hours: Standard business hours with flexibility to accommodate later field visits, as needed
• Setting: Onsite and Field Work

Responsibilities:

• Provide case management services to an assigned caseload of residents and families
• Coordinate referrals and follow-up for medical, behavioral health, substance use, vocational, and public benefit services
• Conduct home and field visits to support clients in the community and maintain continuity of care
• Assist clients with daily living skills, supportive counseling, crisis intervention, and advocacy
• Document services accurately and collaborate with the care team to support housing stability and community integration

Qualifications:

• Bachelor's Degree
• Prior case management experience
• Experience working with adults and families in behavioral health, mental health, substance use, or supportive housing settings
• Ability to work independently, stay organized, and manage a community-based caseload
• Strong written and verbal communication skills

Desired Skills:

• Bilingual (English and Spanish)
• Experience working in a scattered-site housing setting
• Familiarity with the AWARDS system
• Experience supporting clients with substance use and mental health needs
• Knowledge of community resources, medical follow-up, and public benefits coordination
