Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources. Minimum Qualifications: Degree: Possess a Bachelors degree in Nursing from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).

Minimum of 3 years' experience in public health nursing, acute care, case management, and/or in-home health care is required; a minimum of 2 years of managed care experience in case management with a focus on inpatient and/or outpatient ambulatory care is preferred.
