Provide navigation to remove barriers to care such as transportation access and routes, planning for arrival times, paperwork completion for healthcare appointments, interpreter services, what to bring to a healthcare appointment, and other planning, information and direct supports needed. POSITION SUMMARY: Perform the duties required to provide services to refugees by promoting access to preventive healthcare services, health education, PRAPARE screenings, follow up, and linkage and navigation services for resources to support wellbeing and healthy lifestyles.
