Work closely with providers in the Collaborative Care Program outpatient clinic as part of the interdisciplinary team to identify eligible patients for the Food Rx program, coordinate enrollment and follow-up, and provide MNT/DSME consults. In this position, you will be joining the Food Rx Program, a national award-winning regionally based program in Baltimore, MD that helps reach individuals in the community with chronic diseases and food insecurity.
