You will work side-by-side with a multidisciplinary team - including counselors, nurses, physicians, pharmacists, social service staff, and Property Management - to connect clients to the full range of community resources they need: primary care, behavioral health, substance use services, benefit programs, educational supports, vocational resources, and social networks. You will explain the purpose and process of case management to clients, conduct comprehensive assessments of strengths and needs, administer appropriate screening tools, and support clients in developing the independent living skills - including budgeting, understanding leases, and paying bills - that promote lasting housing stability.
