Key Responsibilities: Provide compassionate care, emotional support, and companionship to patients and families Assist with personal care, mobility, and daily living activities Monitor, document, and report patient condition and changes to the RN Ensure patient safety and proper use of equipment Maintain a clean, safe, and comfortable home environment Follow care plans, policies, and participate in training and team activities Requires reliable personal transportation and willingness to travel to patient homes throughout North Cook and Lake Counties (including, but not limited to, communities such as Evanston, Glencoe, Arlington Heights, Schaumburg, and Deer Park). Field CNA - Northern Cook and Lake County The field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patients personal needs and to promote comfort.
