Position Responsibilities: Prepare patients for provider visits by obtaining and documenting vital signs, medical history updates, medication reconciliation, and required screenings, including depression, social determinants of health, barriers to care, tobacco use, and other preventive health assessments. The LPN is responsible for patient intake and rooming, clinical screenings, telephone triage, care coordination, medication administration, clinical treatments, patient education, and assisting providers in the delivery of high-quality, patient-centered care.
