The main responsibilities for this role focus on the following key areas: - Interprets and evaluates medication orders prior to entry/verification - Clinically assesses orders for dose, route, frequency, and appropriateness of medication therapy - Monitors orders for drug interactions, adverse reactions, incompatibilities, therapeutic duplication, allergies - Identifies, mitigates, prevents, and documents problems with medication orders or therapeutic regimens, including medication errors or adverse drug reactions - Verifies the accuracy of order entry and of medication products dispensed - Delivers optimal pharmaceutical care by participating in direct patient-care interdisciplinary rounds - Advises nurses, doctors, and patients on the proper and safe use of medications - Narcotic control and assists in record maintenance - Oversees pharmacy technician activities - Assists in developing and implementing departmental clinical and educational programs - Assists in inventory control and proper medication storage - Compounds extemporaneous medications - Prepares IV admixtures as needed. Inspects completed I.V. solutions for accuracy and compatibility - Complies with USP 797 and USP 795 when compounding or overseeing compounded medications - Prepares prescriptions in accordance with pharmacy laws and practice - Maintains proper licensure and educational requirements and provides documentation upon request.
