The Nurse will provide hospital-level acute care in a patient's home and will be part of the NM Hospital at Home team including (depending on each patient's needs) Physicians, Paramedics, Advanced Practice Providers, Pharmacists, Case Managers, Social Workers, Home Health Aides, Physical Therapists, and other operational staff. Supports patient/family/spiritual/cultural needs when assessing the patient and family needs for teaching; identifies the patient''s learning level and readiness, and documents education record.
