In this role, you will conduct targeted outreach calls to patients, families, and healthcare organizations — educating them on palliative care and VyncaCare's services, completing structured eligibility screenings to identify patients with chronic illness and functional decline, and scheduling those who qualify for an initial visit with a nurse practitioner. Act as a liaison between patients and the broader care team, directing calls, texts, and emails to the appropriate individuals or departments and responding to patient questions and concerns in a timely, supportive manner.
