Functioning as an advocate for the patient, works to identify initial goals for rehab and outcome achievement potential and is responsible for determining how available resources can be used and making referrals to appropriate community, financial, support and advocacy resources, verifying available healthcare benefits and is the primary point of contact for all insurance company case managers, providing communication as required for precertification of patients' hospitalization and treatment to all involved parties to facilitate continuity of care for each patient and ensures a smooth transition to the rehab program. Overview: Responsible for providing case management services to all patients referred to WakeMed Rehab as well as marketing activities to monitor and develop relationships with all referral sources.
