Works with Social Services, the community Coordinator Resident Services, residents, families, and nursing team members to coordinate a smooth transfer from independent living community into Health Services. Educates on clinical matters as needed in the household and neighborhood and serves as a clinical mentor to the clinical leaders for regulatory compliance, general operations, infection control, wound management, and team member education.
