Crossroads Health, a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Cleveland, serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services-no matter one's ability to pay. Additionally, the Prescriber will: Provide general comprehensive primary medical care services to individual patients across the life span in the office setting including behavioral health assessment, medication evaluation and medication management, and prescription of medication in compliance with Ohio state regulations using a trauma informed approach.
