This role provides both direct and indirect support to individuals and families affected by mental illness, collaborating with individuals, families, and primary care providers to design and implement service plans centered on recovery goals while maintaining strong, cooperative relationships with community partners and other professionals. Case Managers perform a variety of duties including assessment and service planning, case management, emergency services, and service coordination, and may include frequent court appearances as well as identified special assigned responsibilities.
