Coordinate with other CCC programs supporting patients in a continuum of services such as Hepatitis C treatment, withdrawal management, residential treatment for substance use disorders, specialty mental health, and overdose prevention programs. Along with the care team, support patients in achieving health resiliency and engagement by utilizing techniques such as shared decision making, motivational interviewing, continuity relationship building, health education, and preventive care appropriate to patient characteristics.
