Partner closely with the Director of Product Management and the PM, NavigatEHR to keep the practice and ACO experiences aligned — NavigatEHR surfaces ACO insights and metrics as they matter to the practice, while IlluminatEHR owns the full ACO operational workflow; the insights surfaced by IlluminatEHR should also directly inform and measure the care programs (CCM, BHI, APCM, CoCM, RPM) managed through OrchestratEHR. Build IlluminatEHR into the system ACOs run their business on, not just a reporting layer — the console where ACO teams manage eligibility and attribution, close care gaps in-workflow, capture HCC/RAF risk data as care is delivered, track admission/discharge/readmission activity, and monitor shared-savings performance across skilled nursing facilities, assisted living communities, memory care, and CCRCs.
