JOB SUMMARY: The Clinician in a Post-Acute Care setting provides comprehensive evaluation, treatment, and care management services to patients transitioning from acute hospitalization to rehabilitation, skilled nursing, long-term care, or home-based care environments. The clinician collaborates with interdisciplinary teams, including physicians, nurses, therapists, case managers, and caregivers, to develop and implement individualized care plans that promote recovery, independence, and quality of life.
