Job Description:The primary responsibilities of the Assessor are:<div><ul><li>Provide substance use Level of Care Assessments via assessing the patient's needs and determine appropriate level of care based on placement criteria established by the American Society of Addiction Medicine (ASAM)</li><li>Maintain patient chart via electronic monitoring records with accurate and complete documentation</li><li>Abide by regulatory compliance, local, state and federal laws including HIPPA and 42 CFR</li><li>Coordinate continuum of care activities for individuals seeking assessment, provide access to available ancillary services, and ensure optimum compliance with external agencies and referral source requirements</li><li>Facilitate appropriate funding arrangements </li></ul></div>Requirements<ul><li>Experience providing level of care assessments to SUD and co-occurring population required and/or experience providing screening and assessments per ASAM guidelines. </li><li>Candidates will possess a minimum of Masters Level Degree in a related field</li><li>Knowledge of funding sources such as Medicare, Medicaid, Commercial/Private insurance</li></ul>MVP strives to support and encourage a working atmosphere of positivity and compassion for both our clients and our staff.MVP Recovery is the most comprehensive recovery program in the region.The MVP Recovery program offers a dynamic patient-centered approach to SUD treatment. <div>Energy Assessor Location: Burlington, NJ </div><div>We are seeking a skilled Energy Assessor to perform daily residential energy audits in alignment with program requirements and under the direction of the Field Coordinator. • Works independently in scheduling of and completing required tasks in the homecare setting.
• Completes annual LOC assessments accurately and within required timeframes.
• Assists in developing care plans based off of information obtained during assessment.
• Completes and meets all requirements to maintain certification annually.MINIMUM REQUIREMENTS• Bachelors Degree in healthcare related or behavioral science field or licensed clinical or licensed certified social worker, or licensed registered nurse.
• One years experience working directly with the elderly or physically disabled in an institutional or community setting.
