Conduct client visits in homes, assisted living facilities (ALFs), skilled nursing facilities (SNFs), physician offices, hospitals, and other care settings as assigned; assess physical, cognitive, emotional, environmental, and care-plan needs during each visit. The Care Manager delivers high-quality, client-centered aging life care services in the field while maintaining same-day documentation, accurate billing support, and proactive communication with families, aides, physicians, and the supervisor.
