Facilitate timely follow-up for patients at elevated risk of severe obstetric complications, including hypertensive disorders of pregnancy, cesarean or surgical complications, postpartum hemorrhage, infection, preterm birth, and positive mental health screenings. You will collaborate with physicians, advanced practice providers, Care Management teams, Perinatal Navigators, and Care Team Assistants to make each patient's care journey safer, more connected, and more supportive.
