Collaborate with a multidisciplinary team that includes heart transplant APPs, VAD/MCS APP coordinators, inpatient APPs, outpatient transplant RNs, attending physicians, transplant surgeons, discharge coordination, outreach clinic teams, and pre-transplant evaluation staff. This role supports adult post-heart transplant patients in the outpatient setting, partnering closely with APPs, physicians, surgeons, inpatient and outpatient teams, and caregivers to promote continuity, safety, and long-term success after transplant.
