The coordinator provides education, connects families with hospital and community resources, facilitates referrals, and addresses barriers to care to improve patient outcomes, reduce readmissions, and promote continuity of care following hospitalization. Purpose & Scope: The Women & Infant Perinatal Care Coordinator is responsible for coordinating comprehensive care planning, education, and transition of care for maternal, newborn, and high-risk perinatal patients.
