Our organization includes The LGAR Health and Rehabilitation Center, a 60 bed Skilled Nursing Facility, as well as Serenity at LGAR, a 40 bed Personal Care/Memory Care Facility in the eastern suburbs of Allegheny County. Coordinate all aspects of discharge planning to include NOMNC, ABN, associated appeals, Home Health Care referrals, referrals to transitions programs as appropriate and conferences with resident, family, and appropriate staff.
