Highest licensure by a State or National regulatory board that authorizes independent clinical practice that is current, valid, unrestricted and in good standing in at least one of the following: clinical social work, licensed marriage and family therapist, licensed professional counselor, or clinical psychologist that meet the DoD requirements. Complete and maintain appropriate documentation related to: intake paperwork, screening tools, risk assessments, safety plans, clinical counseling notes, individualized service plans, clinical case management, collateral notes, contact details, and other documentation as required by FAP policy.
