Working closely with the Market-based Population Health teams, primary care providers (PCPs), networked partners, and Performance Management at Privia Health, the Population Health Quality Coordinator will provide execution on defined Value-Based Program Performance Metrics, care coordination, and support for Privia’s at-risk patients. Make appropriate referrals to internal care team, payer programs, community resources, home care, and disease management programs.
