RESPONSIBILITIESProvide services that are client-centered, trauma-informed and resiliency-focused Ensure client confidentiality (as outlined in CGS 52-146K and PCC's confidentiality policy)Ensure compliance with funding requirements and contracts and PCC's policies and proceduresWork collaboratively with PCC staff, CT Coalition Against Domestic Violence (CCADV) staff, and other community service providers to address client needsComplete intakes, assessments, and screenings to determine housing eligibility based on HUD and CT Department of Housing (DOH) guidelinesComplete survivor-centered, strengths-based housing plans that identify barriers to housing and set a path to housing stability; reviewed and updated monthlySupport survivors in designing and implementing safety plans and housing risk assessmentsProvide home visits or community visits, safety permitting, at a minimum of once per month, with each enrolled survivor Provide case management services focused on increasing income, accessing long-term housing subsidies, establishing and/or expanding family and community-based supports, and assisting in increasing access to community servicesDevelop and/or maintain a relationship within the homeless service systemAssist in program development and quality assurance activities Assist survivors with locating available housing, the leasing process, landlord mediation, and tenant/landlord rights and responsibilitiesSubmit requests for rental payments for enrolled survivors Assist with protective/restraining orders and other court mattersArrange for emergency medical services or law enforcement services as neededProvide/arrange transportation for clients as neededOther duties as assigned DOCUMENTATIONMaintain accurate and up to date files and records to include required documentation and confidential case management notesComplete and submit:Timesheets and monthly Personnel Activity Reports (PARs) Annual reports as neededRRH HMIS quarterly reports Document all client and program data in EmpowerDB within 48 hours of contact TRAINING & PROFESSIONAL DEVELOPMENTAcquire and maintain certification as a Domestic Violence CounselorAttend local CAN meetingsParticipate in the annual Point-in-Time countAttend CCADV, HUD-related and DOH trainings and workshops related to housing GENERAL AND RELATED RESPONSIBILITIESAssist in additional agency operations as requestedActively participate in staff trainings and meetingsAssist in fundraising and community awareness activitiesRecord and compile statistics as required Case management utilizes a Housing First philosophy, includes trauma-informed, resiliency-focused and survivor-centered approaches to stabilize housing, rebuild self-esteem, develop essential life skills, and establish financial independence.
