Coordinating outreach to community service agencies to achieve consumers' goals and meet service needs.*Completing documentation, data entry, and case noting related to consumer contact including emails, phone calls, meetings etc. in company databasesPlacing online orders for household and furniture items, tracking orders, providing updates to consumer's teams, and troubleshooting missing items as neededPicking up and delivering orders at local stores and moving companies - use of own vehicle, proof of car insurance and valid driver's license are required for the position Provide exceptional customer service to case managers and consumersOur team assists individuals with disabilities in living their own most independent lives, by removing barriers and promoting choices. Duties include but are not limited to:Coordinating Transitional Services Referrals to ensure individuals are moved from licensed facilities or other dwellings to community-based living situations with appropriate furniture, household items, and moving services.
