PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team). You'll collaborate closely with physicians, nurse practitioners, social workers, therapists, dietitians, home care staff, specialists, hospitals, nursing facilities, hospice providers, and community partners.
