The Outreach Support Specialist (OSS) plays a critical role in supporting the Outreach team by managing incoming leads, completing timely patient outreach, coordinating intakes, maintaining accurate patient information, and assisting with the development of Giving Home Health Care’s patient pipeline. The OSS should allow the Outreach Coordinator to spend more time in the community generating relationships, leads, referrals, and transfers, while the OSS ensures those opportunities are promptly contacted, organized, scheduled, documented, and moved toward completed intakes.
