The Case Manager provides community-based case management services and support to Individuals with substance use disorders (SUD) and co-occurring conditions in the following areas: Self-Advocacy, Education, Community Awareness, Medication Support, and Management, using a recovery-oriented and harm-reduction-informed approach. CNS provides comprehensive integrated health services in partnership with several community organizations, and uses a patient-centered approach to identify, support, and promote the overall health of children, adolescents, adults, and older adults.
