Provide emergency care, as needed, for all patients enrolled in the CBOC, including referring patients for emergency primary care treatment, psychiatric medication evaluation, or coordinating the admission of patients to an inpatient psychiatric unit. You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.
