The role of the pharmacist in this specific area includes but is not limited to: working with a multidisciplinary team to optimize patient care, participating in daily rounds, serving as a drug information resource for oncology/hematology-related questions, conducting education sessions with nursing, developing and implementing new chemotherapy-related protocols, evaluating and entering chemotherapy protocols, verifying outpatient infusions, investigational drug services, pharmacokinetic dosing, onsite order verification, pharmacy surveillance and error reporting, profile reviews, code blue response, pharmacy consults, developing processes to prepare patient-specific formulations, etc. The Pediatric Hematology /Oncology Pharmacist provides pharmaceutical care including computer order entry, drug distribution, evaluation of medication orders for appropriateness of drug, dose, frequency and adequacy of monitoring, evaluation of drug interactions and allergies and therapeutic monitoring of drugs to patients across our pediatric hospital.
