Certified Nursing Assistant – Home Care Position Summary: Provides personal care services to terminally ill patients and performs related tasks in the patient/family home, nursing home, and/or assisted living facility in accordance with the plan of treatment as assigned by the case manager. Must be able to perform job-related duties which require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, partial or complete assistance with activities of daily living without assistance from another health care worker or significant other.
