POSITION SUMMARY:Responsible for individualized implementation of a broad array of early intervention services. All NAPPR employees are expected to demonstrate integrity and focus on NAPPR's mission throughout all aspects of their work. <h1>DUTIES AND RESPONSIBILITIES / ESSENTIAL JOB FUNCTIONS:</h1><ol><li>Conduct evaluations and assessments with other team members to determine a child's eligibility for services.</li><li>Conduct vision/hearing screenings.</li><li>Facilitate and participate in the development, review and evaluation of the Individualized Family Service Plan (IFSP) in various teams in order to ensure that families receive comprehensive, individualized services that are responsive to family and child needs.</li><li>Provides Service Coordination and Developmental Instruction, individualized early intervention services in home/community-based settings to families who have infants/toddlers with or at risk for developmental delays in compliance with federal, state, and organizational requirements.</li><li>Develops and maintains relationships with all stakeholders by maintaining frequent appropriate communication focused on responding to and meeting the needs of the family.
